It was installed and tested one time when new, then put back in the box -- the other card never failed so this backup was never needed! Only the card, remote, box, and accessories you see in the pictures are included. I believe everything that originally shipped in the box is in there, but I have no way of verifying that. Because this is a delicate vintage computer item, I am selling it as-is and as shown, and will not be accepting returns unless it is damaged in shipping and doesn't arrive in the same working condition described and that these pictures show. It will be packed well. Note: I have a second one of these cards in used condition -- it is the one that this card was a backup for. Let me know if you are interested in that one as well! It doesn't have all of the accessories and box like this one, but the card is in identical shape.
