Major changes are on the way, such as the Americans with Disabilities Act (ADA),' and the increasing emphasis upon industrial hygiene and safety instruction in some occupational medicine residency training programs. In addition, there are the expanding demands placed upon the occupational medicine specialist by attorneys, judges, insurance carriers, and employers to support theories of cause and effect, placement decisions, and medical restrictions with facts, not fantasies or guesses, about workplace exposure scenarios. Gone will be the days when the physician (specialist or otherwise) can utilize poorly conceived and undocumented, second or third-hand assertions about the workplace to buttress major decisions which will affect the worker, the employer, and other parties so profoundly. Under ADA, moreover, the physician can expect to play an even more important role in deciding worker fitness, and the nature and scope of reasonable accommodations for disabled workers, while simultaneously helping the employer to define the essential functions of a given job. Performance of these roles cannot be achieved over the telephone or by sitting in one's office. They must be achieved by visiting and becoming familiar with the details of the workplace as a total physical and functional environment in which the worker-patient spends a large percentage of time. The capable performance of these expanded duties should propel occupational medicine onto an even higher plane of respect than it enjoys today. None of these laurels will accrue, however, unless all practitioners of occupational medicine take these challenges seriously.
